COMMUNITY-BASED HYPERTENSION CONTROL PROGRAM IN RT 01 RW 01 JUMERTO VILLAGE, PATRANG DISTRICT, JEMBER REGENCY

Productive age is a risk group that prone to hypertension and increases with age. Management of plays role in maintaining the stability patient's medical condition slowing occurrence complications. However, implementation disease management program has not been carried out optimally. This research was quantitatively, quasi-experimental type using one-group pretest-posttest design approach. Activities were four stages, namely situation analysis, plan formulation, implementation, monitoring evaluation. Evaluation systolic diastolic blood pressure knowledge participants done before after program. The intervention results showed there significant differences systolic, pressure, public (p<0.005). Community based programs have proven be effective increasing clinical conditions people hypertension. Therefore, increase knowledge, attitudes, skills control support are needed by health workers, cadres families.
